Use: Have your respondents answer your questions by checking a box.

Instructions

  1. Find the Checkboxes widget.
  2. Drag and drop it into the Body container.
  3. Click Edit.
  4. Enter your question in the Label section.
  5. Enter your choices in the Choices section.
    1. You can add "Other" as an option where your respondents can type in their own answer.
  6. Optional: Make answering your question required by selecting Required Field.
  7. Save.
